In order to consider any proposal for research involving human subjects, the IRB requires the following:
- Download and complete the appropriate form from this site. Only Lyndon State College approved forms are allowed. Be certain to describe your research purposes and processes with sufficient detail that the goals and methods will be clear and complete. Do not attach a copy of a research proposal; Federal Law requires this petition to be a stand-alone document.
- Provide your complete Informed Human Subjects Consent (and assent, if necessary) package. See the link below for assistance.
- You may submit materials electronically to the Chair of the IRB, but the IRB will require a single approved copy of all materials with original signatures be placed on file in the Associate Academic Dean's Office prior to the initiation of the research.
